This page details how to add a feedback activity to a course



The feedback activity module enables a trainer to create a custom survey for collecting feedback from participants using the course. They are useful for helping to improve course content. 


After selecting the add new activity option in the main course page, select 'Feedback' from the list of activities and click 'Add'. You will then need to give your feedback activity a name and optional description.


Under the 'Question and submission settings', you can choose whether to make the feedback from users anonymous using the drop down 'Record user names': 


Once the applicable sections have been completed, click 'Save and return to course'. You are now ready to add content to your feedback activity. On the main course page with editing turned on, click on the feedback activity name: 


When you are in the feedback activity, click the 'Edit questions' tab. You can then choose the type of question to add to the feedback activity: 



For this example, a long answer question has been added. Choose whether the question is required or not, type the question and then decide how much space you will allow for the user to give their feedback. Once complete, click 'Save question': 


Another example of a feedback question is a rated multiple choice question. When adding the multiple choice values, ensure you add one answer for per line in the 'Multiple choice values' box: 



You can also add questions to a feedback activity from a template already created in another course from the 'Templates' tab. You can view the analysis of the answered questions under the 'Analysis' tab and view user responses on the 'Show responses' tab.
